Each line
Each line says who made the change, in which role, on which ship and at what time, and which record it touched. It is read by date, ship and module.
Accountable and transparent: who did what, and when, stays on the record, ready to show an auditor.

A person's role is kept as it was on that day. A chief officer promoted next year still reads as chief officer on this year's lines.
When a record is deleted, the deletion gets its own line and the earlier lines stay. No screen can edit or remove a line.
Every job card has its History: each completion and unplanned repair, with its date, its note and the materials used.
Assignments (Sea Services) and Separations hold each person's service. An appraisal shows every step in its History, back-steps included, with who and when.
A finding shows when it was raised and when it was closed. Each corrective action shows who did it and who verified it.
A permit says who issued it, a drill says who sealed it, and each seal keeps its who and when.
Audits, incident reports and near-miss reports are sealed by the office. Opening one again asks for a sentence, and that sentence stays on its trail.
Reopening a closed finding or taking back a verified action asks for a reason too, kept with who and when.
